Why did Dr. Poormehr decide to establish the South County Private Medical Care practice?
During my 17 years of practicing family medicine, I have been dedicated to serving the medical needs of all my patients. The most gratifying aspect of my practice has been the relationships that I have developed with my patients and their families. Many of my patients have been with me for years. As a result of my dedication to serving the need of all my patients, my practice has grown very large. Despite working harder and harder, it has become increasingly more difficult to deliver comprehensive care in a timely and unhurried manner.
How much is the annual fee?
Fee Schedule, Based on One Year
Initial Adult Member $1,950.00
2nd Adult Member $950.00
Additional Members (age 5-15)Each $500.00
How many patients will be enrolled in the Private Medical Care practice?
There is no specific number. What is most important is that the number will be kept small enough so that each patient gets the highest level of service possible from Dr. Poormehr.
I need more time to make a decision.
Dr. Poormehr will be able to continue to see you if you have PPO insurance or prefer to render cash for the visit, until the Private Medical Care practice is full. When the practice is full, Dr, Poormehr will dedicate 100% of his office time to his Private Medical Care practice patients.
When will the Private Medical Care practice be closed? How much time do I have to make a decision?
I am not certain when, but enrollment is going well and we think that the Private Medical Care practice will fill up soon. When the practice is full, Dr. Poormehr will dedicate 100% of his office time to his patients in the Private Medical Care practice.
Does the Annual Fee cover regular office visits?
Yes. In addition, unlike most other Private/Concierge/Boutique medical practices, all of your acute injuries and urgent visits are included.
Does this change mean that I will not be able to see Dr. Poormehr after April 2008?
Dr. Poormehr -will continue to sec PPO and cash paying patients until his Private Medical Care practice enrollment is complete. Although we do not know an exact date, you will be given adequate notice.
I have a deductible. Can the Annual Fee be applied to my deductible?
Probably not. The Annual Fee is for the services listed in the announcement letter and brochure that are not considered insurance covered services. However, as a courtesy to you we can try to satisfy your deductible with every visit that you come in for.
Do I need to have medical insurance?
YES, for major medical, specialist care and hospitalizations.
Why do I have to pay an Annual Fee when I have insurance?
The Annual Fee is for the Private Medical Care services listed in the announcement letter and brochure that are not considered insurance covered services.
I only come to the doctor a few times a year. Why should I enroll in the Private Medical Care practice?
You need to set your health and wellness priorities. An additional $162.50 per month may seem like a lot of money but many people spend even more on much discretionary expenditure each month. You deserve the kind of health care that Dr. Poormehr is offering you. We encourage you to make your health a high priority.
What happens when I need to see a doctor and Dr. Poormehr is not in the office?
You will have access to Dr. Poormehr by cell phone 24/7. He can still guide the health care decision making, even when he cannot see you in the office. It is unlikely that you will not be able to reach Dr. Poormehr, However, if he is unreachable, then he will select a physician to cover him until he can return your call. That physician is directly answerable to him.
If I become a Private Medical Care patient, can I just walk in whenever I need to be seen?
Dr. Poormehr will see you if you walk in, but you will have to wait until he first sees the patients who have made appointments. It is preferable to make an. appointment so that you do not have to wait.
I am an elderly patient, why should I enroll in the Private Medical Care practice?
The older you get the more likely you are to need to see a doctor frequently. In our Private Medical Care practice you get open access to the doctor. You get the care that you want. You get the care that you deserve.
What happens if I move? Is anything refundable?
The Annual fee can be refunded on a prorated basis, but not after the Annual Wellness Exam has been done. Your records will be copied and send to your new physician as soon as we receive your written authorization to do so.
